The Details:

• Complete the GearMax® 2008 Promotion Participation Request Form below.
• E-mail the Team (or School) Logo to be embroidered to: info@GearMax.com.
  Logo must be in an acceptable graphic format.
• A formal approval of participation and participation packet will be sent to the designated contact. This will be sent in PDF format. Adobe Reader is required to open the documents.
• Obtain orders for at least 24 GearMax® M Series or S Series Back Packs. There is no limit to the maximum number of GearMax® Back Packs that can be ordered under the promotion.
• No discounts are available under this promotion. The prices for GearMax® products under the promotion are: M Series $39.95 each; S Series $34.95 each.
• Send your completed promotional order form showing the number of packs to be ordered to info@GearMax.com. GearMax® will compute the shipping charges and send an electronic invoice to the submitter.
• Send a copy of the invoice and payment in full to:

• Each submitted Team Logo will be graphically configured for embroidery and will require a written approval before the order goes to production.
• The order will take approximately 3-6 weeks to completion and shipment. Shipment time will vary according to location.
• GearMax USA Ltd. will donate $10 per bag to the organization designated on the GearMax® 2008 Promotion Participation Request Form. Payment will be sent the same week as the shipment.
